| A living will is a written document that
explains what care you wish to receive (or not receive) under certain
circumstances. This document, known as an advance directive,
expresses your wishes should you become unable to express your wishes at
some time in the future.
It is important that you have a living will. It assures that
your wishes are known to your medical care provider and provides
guidance to your loved ones in a difficult time.

In Pennsylvania, there is more than one correct
way to write a living will. For your convenience, we have
provided a form for you to use for your living will. Download

It is required that you sign your living will. If you are unable to
sign it, you must have someone else sign it for you with two witnesses.
You do not have to notarize your living will in Pennsylvania, but if it
is to be used in another state, you may wish to have it notarized. It is
also suggested that you date the document.
Under Pennsylvania law, you may revoke your living will at anytime.
Tell your medical care provider that you wish to revoke it.
Additionally, someone who saw or heard you revoke your living will may
inform your medical care provider.
